Whitley County Home Health Agency is a non-profit program of the Whitley County Health Department and has been serving citizens of Whitley County since 1969.
Our agency employs a variety of professionals ranging from RNs to Certified Nurses Aides. These highly skilled individuals have a combination of over 150 years of health care experience and provide a multitude of diverse services across Whitley County.
The Whitley County Home Health Agency continues to be dedicated to promoting the physical and emotional well-being of the patients we serve. Because of this commitment, we strive to demonstrate our belief in the dignity and worth of each individual. We recognize that every human being has personal rights which must be respected.
Home Health services are available upon physician’s referral. Referrals are based upon the patient’s home care needs, homebound status, physician’s order and the type of services required.
Whitley County Home Health Agency can provide a service or combination of services in the home, all under the direction of a qualified physician. Working with a physician, our highly skilled staff will plan, coordinate and provide care tailored specifically to the needs of the individual patient through traditional Home Health.
Staffing consists of Skilled Nursing, Home Health Aides, Physical Therapists, Occupational Therapists, Speech Therapiests and Medical Social Worker. A Certified Nutritionist/Diabetes Educator is also available.

Social services are provided by a qualified social worker to assess a patient’s social, emotional, environmental, financial and support needs as related to his/her medical condition. Social Services can provide the patient and family with education in many areas, including but not limited to: counseling for long-range planning and decision making, community resource planning, referrals, and assistance with understanding medical bills or other financial issues. Social Services are provided under the direction of a physician.
Quality personal care, which can include: assistance with bathing, grooming, nail/foot care, skincare, and bed linen change provided by our Home Health Aides.
Whitley County Home Health Agency is licensed to provide Medicaid Waiver Services. This program allows individuals who qualify for nursing home placement to stay at home with the help of the agency’s staff by providing case management, attendant care services (housekeeping, meals, laundry, shopping, and personal care), and respite. Occupational Therapy specializes in the assessment and treatment to regain the skills for the job of daily living, home safety, and adaptive equipment assessment and training. Our goal is to work closely with patients, families, and caregivers to establish a home exercise program in order for them to be as independent as possible in the least restrictive environment.
Physical Therapy provided in the home is an essential component of restoring function and independence following a recent illness, surgery, hospitalization, or other debilitating events. Physical Therapists are specialized in functional mobility as well as musculoskeletal and neurological conditions. Initially, your therapist will conduct a thorough evaluation of your current mobility needs and work with you to develop a plan and set goals that are applicable to your daily function.
